- How Can I Obtain My Baby's Birth Certificate?
- During your stay at Chilton Memorial Hospital, our staff has obtained the information necessary to process your baby's birth certificate. This is sent to the State of New Jersey within three (3) days of your baby's birth as required by law. The State then will send the original Birth Certificate to the Pequannock Township Health Department.
Within a few weeks of your baby's birth you will receive a letter from the Township informing you that the Birth Certificate is now available to you, and may be purchased from the Township by:
- Sending in the required fee; and
- Enclosing a stamped, self addressed envelope; and
- Submitting copies of acceptable forms of identification
- A valid photo drivers license or photo non-drivers license or
- A photo ID and an alternative from of ID with address, or
- Two alternative forms of ID with address

- Does Chilton Offer Newborn Photography?
- Chilton Memorial Hospital offers the latest in digital photography, and gives you the option of safely accessing your new baby's photograph and information on the Internet. You may also order photographs of your baby. This service is optional, and there is no obligation to have your baby photographed or place an order. Our staff has information about the newborn photography service, so ask your nurse about this service when you arrive in the Mother and Baby Care area.

The New Jersey Hospital Care Payment Assistance Program, also known as Charity Care Assistance, is free or reduced charge care provided to patients who receive inpatient and outpatient services at acute care hospitals throughout the State of New Jersey. Hospital assistance and reduced charge care are available only for medically necessary hospital care. Some services such as physician fees, anesthesiology fees, radiology interpretation, and outpatient prescriptions are separate from hospital charges and may not be eligible for reduction. Hospital care payment assistance is available to New Jersey residents who:
- Have no health coverage or have coverage that pays only for part of the bill: and
- Are ineligible for any private or governmental sponsored coverage (such as Medicaid); and
- Meet both the income and assets eligibility criteria.
This program is not insurance. Although each situation is different, if you have current determination of acceptance into the program for 100% coverage by the Hospital Assistance Program, the Hospital cannot bill you for medically necessary hospital services such as clinic visits, blood work, respiratory care and diagnostics; EKG, stress testing; physical therapy; consultations with Social Services, Clinical Dietitians, and Nursing services that are provided by employees of the Hospital. If you have been partially approved for Hospital Assistance, the hospital will bill you for that portion of the service as stated on your determination.
This program does not cover the costs of Physician services (e.g., the final interpretation of a diagnostic test, X-Ray, or CT scan, and ED physician services), office visits or specialty referrals. These physicians are not employed by Chilton Memorial Hospital, and you may receive a bill from these physicians. Also, tests and procedures performed outside of the Hospital, elective surgeries or procedures that are not medically necessary are not covered Medications are not covered under the Hospital Assistance Program. At Chilton Memorial Hospital we make every attempt to help you get the medications that you need so that you can get better and stay healthy. Our clinic staff will work with you to apply for free or reduced price medications from drug companies and state run programs.
To obtain an application for the Hospital Assistance Program, simply ask a member of the Chilton Memorial Hospital health care team or call the Patient Accounting department at 973-831-5113. Chilton Memorial Hospital will make a written determination of your eligibility for The New Jersey Hospital Care Payment Assistance Program (Charity Care Assistance) with in ten working days of receipt of a completed application. If all required information is not received the notification will inform you of the documents that are missing. Please remember, if you have been approved for Hospital Assistance at Chilton and you seek care at another Hospital, you must apply for The New Jersey Hospital Care Payment Assistance Program (Charity Care Assistance) at the new hospital.

- When Will I Receive My Child's Social Security Number?
- Chilton Memorial Hospital electronically submits information for all newborns to the Social Security Administration within a few days of your child's birth. It can take up to six months for the Social Security Administration to process the information.
